View Detailed Provider Agency Information

Use this tutorial to view detailed information for your provider agency.

Complete the following steps to View Detailed Provider Agency Information:

1. Select the Provider Agency Search menu option on the Provider Services menu.  The Provider Agency Search page displays.

As a provider, search criteria display automatically for your provider agency and the following information is read-only and cannot be changed:

• Level Of Care

• Provider Agency Name

• Initiative

• As Of Date

2. Click the Details link in the Provider Agencies table.  The page refreshes and displays Provider Agency Detail information.

3. View the detailed provider agency information displayed.
