Search for a Clinician

Use this tutorial to search for a clinician and/or filter the list of clinicians displayed on the Clinician Search page.

Complete the following steps to Search for a Clinician:

  1. Select the Clinician Search menu option on the Provider Services menu.  The Clinician Search page displays.

  2. Complete information in one or more of the following controls to search for a clinician and/or filter the list of results displayed:

  3. In State Ref, enter the specific state reference number on which to search.  You can enter partial information to widen your search.

  4. In First Name, enter the name of the client on which to search.  You can enter partial information to widen your search.

  5. In Last Name, enter the name of the client on which to search.  You can enter partial information to widen your search.

  1. Click Search to perform a search based on the search criteria specified.  The page refreshes and results found to match the search criteria are displayed in the Clinicians table.

NOTE:  If search criteria are not specified, a list of all clinicians displays in the Clinicians table when Search is clicked.
